Registration Forms for SY 2025-26 (New Students Only)
Fill out and drop off at school main office with registration fee.

Medical and Dental Forms
Dental Exam Form– Required for all new students, and students entering Kindergarten, 2nd grade, and 6th grade.
Eye Exam Report– Required for all new students, and students entering Kindergarten
Physical Exam Form – Required for new students entering PreK–8, and students entering 6th grade.
Allergy Form – Required if your child has allergies that require treatment.
School Medication Procedures and Forms – Required if your child takes medication during the school day.
